Rethink Reconnect Service

The Reconnect Team have been in existence for over 20 years and are funded by Somerset County Council, although we are totally independent.
We support people with memory problems/dementia to remain as independent, safe and happy as possible, in their own home. We can work with clients and their carers/family for a maximum of 2 years, supporting them with paperwork, benefits, home and personal safety, social opportunities etc. Usually, the same member of staff will visit the client weekly, for approximately one hour, which enables us to build a trusting relationship.

All our staff are DBS checked at the enhanced level.

We offer a free service.

Anybody can refer to our service, with the person they are referring’s permission.
